Diario del paciente ostomizado

El Diario del paciente es una herramienta sencilla en su manejo y útil tanto al paciente como al profesional sanitario.
El concepto es la cumplimentación de un cuestionario, que varia en función de la evolución física y psicológica del paciente, adaptándose a las necesidades del mismo.
El diario le va ayudar a tomar conciencia de la evolución de su estoma, tanto desde punto de vista de cuidados, como psicológicos. Al rellenar el cuestionario, usted va a poder reflexionar y motivarse con sus avances ejerciendo un refuerzo positivo en su evolución.
El profesional sanitario va a tener una visión de la evolución de su ostomía de una manera rápida, exacta y concreta, pudiendo centrarse en los puntos más críticos optimizando el tiempo de consulta.
El cuestionario ha sido desarrollado por un grupo de enfermeras expertas en Ostomía.

Idea started with a Coloplast Professional group in a brainstorming in the visit to our factories in Tatabania. Managed locally by Coloplast team.

Description: The patient’s diary is a simple tool in its management and useful both to the patient and to the healthcare professional.

The concept is the completion of a questionnaire, which varies according to the physical and psychological evolution of the patient, adapting to the needs of the patient.

The diary will help the patient to become aware of the evolution of their stoma. By filling in the questionnaire, the patient will be able to reflect and motivate by himself/herself with their progress, exercising a positive reinforcement in their evolution.

The health professional will have a vision of the evolution of his ostomy in a fast, exact and concrete way, being able to focus on the most critical points optimizing the consultation time and providing a good care to the patient.

The questionnaire has been developed by a group of expert nurses in Ostomy.
